Why Not Admit Directly to Respite at a SNF
Medicare defines respite as a break for primary caregiver to prevent burnout, not as a quick discharge solution. Through working with Noble, we can achieve the same-day discharge while staying within Medicare guidelines. In addition, the group homes we work with have the potential to be permanent homes for our patients. Many patients prefer the home-based setting and individualized care that our group home partners offer.

What We Do
Our Program Includes:
• Same-day placement in a vetted group home for up to 5 days.
• Affordable daily respite rates, often covered by Noble’s Non-Profit Foundation.
• Hospice-level comfort care in a warm, residential environment.
During the 5-day stay, our team helps families:
• Find permanent care placement.
• Complete ALTCS applications.
• Navigate emotional and logistical needs with high-touch social work support.
Who Qualifies
• Must meet hospice criteria after bedside evaluation.
• Social worker interview to assess care setting, finances, and placement needs.
Pre-discharge coordination includes:
– New move-in paperwork
– TB test needs
– Durable medical equipment setup
– Medication orders
– Transportation arrangements
– RN arrival support
