Registered Nurse

Noble Hospice provides a trained hospice RN Case Manager as needed to visit patients. Typically, they will visit the patient a minimum of twice a week to assess and manage care. Noble Hospice also has 24/7 access to on-call RN’s to triage and dispatch when there are acute issues.

- Evaluates patients and family members needs.

- Provides education for patients and their family members.

- Administers medication provides wound care, and orders any other specialized medical equipment that is needed for comfort care.

- Monitors and acts as a “Case Manager” for the patient’s health, vital signs, and symptoms.

- Coordinates the unique patient’s plan of care with the entire hospice team.

Hospice Aide

Hospice Aides are local heroes that make a positive impact on the lives of the sick, disabled, and elderly.

- Providing personal care

- Provide assistance with bathing and grooming

- Assisting the patient to maintain hygiene

- Helping the patient transfer to various locations around their home safely

- Ensuring the patient has the necessary supplies for care

- And much more

Social Worker

Medical social workers create long-range plans, such as advance care planning and educate both patients and their families about community resources and support.

- Assessing social and emotional factors

- Advanced care planning

- Community resource planning

Spiritual Care Coordinator

Spiritual Care Coordinators (Chaplains) are provided to assist with making patients comfortable and at peace, while providing family members with a pillar of support during this difficult time.

- Chaplains are available for spiritual and emotional counseling for both patients and families.

- Bereavement counseling services including phone calls and in-person visits to family members.

- Grief support groups for members of the community to know they are not alone in the journey.

Community Liaison

Marketing Liaisons are the first point of contact when a patient is referred to hospice or palliative services. They help educate the community on the benefits our services have for our patients and support the clinical team in any way necessary.

- Represent Noble Hospice in the community

- Educate business partners of Noble Hospice's services

- Act as a first point of contact for the admission process

- Support the clinical team as necessary

Patient Activities:
Volunteers play a vital role in bringing comfort and joy to our patients and their families by engaging in meaningful activities such as reading aloud, writing letters, playing games, and offering companionship. They may also participate in therapeutic experiences like pet therapy and music therapy, or help patients express their creativity through scrapbooking and other crafts. These heartfelt interactions provide emotional support and brighten the days of those in our care, making a profound difference in their lives.

Additional Activities:
In addition to providing companionship and engaging in creative and therapeutic activities, volunteers also assist with a variety of household and administrative tasks. They help with running errands, arranging flowers, walking pets, and offering emotional support to both patients and their families. On the administrative side, volunteers participate in bereavement calls and assist with light office duties, providing valuable support to ensure the smooth operation of our care services.
